About the Role: We're seeking a Mental Health Practitioner to support our growing organization. A Mental Health Practitioner is responsible for providing treatment programs and support to patients in the comfort of their own home. The right candidate will be capable of working in a fast-paced environment, using their knowledge of psychiatry to assess patients and create individualized treatment programs and support plans. A successful Mental Health Practitioner will be detail oriented and able to work independently, using organizational skills to manage their schedule and prioritize tasks on a daily basis.
Responsibilities:
Conduct comprehensive assessments to develop personalized treatment plans.
Provide individual and group therapy sessions using evidence-based practices.
Monitor client progress and adjust treatment plans as necessary.
Collaborate with a multidisciplinary team to ensure holistic care.
Maintain accurate and timely documentation of client interactions.
Educate clients and families on mental health issues and coping strategies.
Participate in regular team meetings and professional development opportunities.
Advocate for clients' needs and connect them with community resources.
Requirements:
Master's degree in Social Work, Counseling, Psychology, or related field.
Current state licensure as a Mental Health Practitioner (LPC, LCSW, etc.).
2+ years of experience in a clinical setting preferred.
Strong understanding of mental health disorders and treatment modalities.
Excellent communication and interpersonal skills.
Ability to work collaboratively in a team-oriented environment.
Compassionate demeanor and a commitment to client-centered care.
Proficient in electronic health record (EHR) systems.
