Pilsen-Little Village Community Mental Health Center, Inc d/b/a Pilsen Wellness Center, Inc. seeks a Mental Health Professional, Program Evening Coordinator to Support patients and psychotherapists in the implementation and continued adherence to each patient's treatment plan which includes: planning and evaluating patients' behavior; guiding patients in grooming and residential cleaning; providing assistance to patients in crisis; and conducting building rounds to ensure patients' safety and security. Administrative duties include weekly team meetings, participating in monthly online training, documenting patients’ progress such as medication changes, treatments, and patients' symptomology, and complying with State reporting requirements.
