Hourly Rate: $20.98 - $24.94

Join our Mental Health Transitional Living Program and help individuals with serious mental illness build independence, stability, and everyday life skills. This is a hands-on role where you’ll support residents in a structured, community-based setting.

Key Responsibilities

  • Serve as Qualified Mental Health Staff (QMHS) in compliance with NYS OMH regulations
  • Manage a caseload and develop Individualized Action Plans (IAPs)
  • Provide daily living skills training (ADLs), coaching, and recovery-based support
  • Monitor and document residents’ mental, emotional, and behavioral health
  • Provide crisis intervention and trauma-informed support
  • Oversee medication administration, documentation, and med counts
  • Coordinate care with healthcare providers, social services, and community resources
  • Facilitate groups and skill-building activities
  • Maintain a safe, structured, home-like environment
  • Complete accurate documentation and Medicaid billing records

Qualifications

Required:

  • Bachelor’s degree in Human Services or related field
    OR
  • Associate’s degree + 3 years experience in a mental health setting

Skills & Experience:

  • Experience working with individuals with serious mental illness (SMI); co-occurring SUD/DD preferred
  • Strong communication and documentation skills
  • Ability to remain calm and make sound decisions in crisis situations
  • Team-oriented with the ability to work independently
  • Compassionate, respectful, and non-judgmental approach

Additional Requirements

  • Valid NYS Driver’s License with acceptable driving record
  • Reliable transportation for client-related travel
  • Ability to navigate stairs and respond to emergency situations
