Job Summary
We are seeking a licensed Mental Health Therapist to join our team! In this role, you will diagnose and treat psychological disorders, teach communication skills, assist patients in addressing dysfunctional behaviors, and guide them through crises. Essential Duties and Responsibilities

• Conducts in-depth biopsychosocial assessments covering medical, psychological, social, and substance use history.
• Utilizes evidence-based tools such as DSM-5 and ASAM Criteria to identify diagnoses and treatment needs.
• Identifies co-occurring disorders and differentiates between mental health symptoms and substance use effects.
• Collaboratively develop treatment plans that address mental health, substance use, and social determinants of health.
• Ensures treatment goals are SMART (Specific, Measurable, Achievable, Relevant, Time-bound) and patient-centered.
• Reviews and updates treatment plans regularly based on progress and patient feedback.
• Utilizes a range of evidence-based therapies, including Cognitive Behavioral Therapy (CBT) and Motivational Interviewing (MI), for mental health and substance use treatment.
• Administers EBT techniques for emotional regulation, distress tolerance, and mindfulness in patients with co-occurring substance use and borderline personality disorders.
• Applies trauma-focused cognitive behavioral therapy (TF-CBT) to address trauma-related mental health issues in patients with a history of substance abuse.
• Utilizes EBT modalities to support members to meet recovery goals.
• Incorporates Relapse Prevention strategies to help patients recognize triggers, develop coping skills, and establish support networks i.e. Matrix.
• Identifies, assesses, and responds to suicide risk, overdose risk, and other psychiatric emergencies.
• Develops and implements crisis and safety plans for high-risk patients.
• Conduct follow-up care post-crisis to ensure stability and adherence to safety plans.
• Conducts individual therapy sessions utilizing therapeutic modalities to address both mental health and substance use issues.
• Facilitates group therapy, including relapse prevention groups, psychoeducation, and process groups.
• Engages patients in a strengths-based recovery approach, focusing on long-term sobriety and mental health maintenance.
• Complete all clinical documentation (e.g., assessments, treatment plans, progress notes, discharge summaries) promptly and accurately.
• Adheres to legal and ethical guidelines in all documentation, including HIPAA and state-specific regulations.
• Utilizes the agency’s electronic health records (EHR) system efficiently.
• Actively collaborates with psychiatrists, medical professionals, case managers, and external agencies to ensure holistic care for patients.
• Attends interdisciplinary team meetings and contributes effectively to case discussions.
• Coordinates referrals for specialized care or services as needed, such as inpatient treatment or housing resources.
• Demonstrates awareness and sensitivity to the diverse cultural, racial, gender, and socioeconomic backgrounds of patients.
• Adapts therapeutic interventions to be inclusive and respectful of patient identities.
• Incorporate trauma-informed care principles, recognizing the impact of trauma on mental health and substance use.
• Educates patients on triggers, cravings, and coping mechanisms to prevent relapse.
• Develop personalized relapse prevention plans that include support systems, self-monitoring strategies, and resources for recovery maintenance.
• Engages families and support systems in relapse prevention efforts when appropriate.
• Administrative duties related to role.

Communication & Coordination

•  Communicate effectively with members, families, providers, hospitals, DSS, schools, and community agencies 
•  Maintain professional communication consistent with HIPAA and 42 CFR Part 2 requirements 
•  Serve as a liaison between members and treatment providers to support continuity of care 
•  Respond professionally and appropriately during crisis situations 
•  Maintain healthy professional boundaries with members and families 

Qualifications

• Master’s degree in counseling, social work, or similar field
• Current and unrestricted state therapy license
• Licensure: Licensed Clinical Social Worker (LCSW or LCSWA), Licensed Clinical Mental Health Counselor (LCMHC or LCMHCA), or equivalent. Provisional license accepted with supervision.
• Experience: Minimum of 1-2 years of experience in providing therapy to individuals with co-occurring mental health and substance use disorders, including experience in crisis intervention.
• Basic Life Support (BLS) and CPR certified
• Excellent communication and interpersonal skills
• Highly organized
