As a key member of our interdisciplinary team, you will play a vital role in ensuring our residents receive the highest quality care and services. Responsibilities:

• Conduct thorough patient assessments, including minimum data set (MDS) assessments, to identify patient care needs and develop individualized care plans

• Collaborate with the interdisciplinary team to implement care plans, providing education and guidance to patients, families, and caregivers

• Monitor and report changes in patient condition, and make recommendations for revisions to care plans as needed

• Participate in quality improvement initiatives, including data collection and analysis, to ensure optimal patient outcomes

• Maintain accurate and timely documentation of patient assessments, care plans, and progress
