Mobile Integrated Health (MIH) Paramedic
Full-Time — Field Clinical Role

Care2U is a fast-growing mobile healthcare company delivering High-Intensity Physician-Led Acute Care (HIPLAC) directly into patients' homes across New York City, Long Island, and Westchester County. Our model pairs field clinicians with telehealth Emergency Physicians, Advanced Practice Providers (Nurse Practitioners and Physician Assistants), and care coordinators to treat acute and urgent conditions in place — keeping patients safely out of the hospital while delivering a patient-centered care experience.
As a Mobile Integrated Health (MIH) Paramedic, you will serve on the front line of Care2U's field operations, delivering acute and urgent in-home care in direct partnership with our telehealth Emergency Physicians, Advanced Practice Providers (APPs), and care coordination team. This role suits an experienced, autonomous paramedic who is comfortable with complex clinical decision-making in a physician-led, home-based care model.
JOB RESPONSIBILITIES
Home Visits
Conduct in-person home visits to assess, treat, and manage acute illnesses and injuries in the patient's home setting.
Treatment & Collaboration
Deliver treatment in accordance with evidence-based clinical guidelines and Care2U protocols.
Collaborate in real time with a multidisciplinary care team — including telehealth Emergency Physicians, Advanced Practice Providers (NPs and PAs), and care coordinators — to manage complex, high-acuity cases.
Risk Assessment
Identify and document risk factors, including medication non-compliance, limited mobility, impaired nutrition, fall risk, and social isolation, and escalate concerns to the care team as needed.
Documentation & Compliance
Maintain strict patient confidentiality in accordance with HIPAA and Care2U policy.
Complete accurate, timely electronic documentation for every visit.
Adhere to Care2U's HIPLAC acute care standards and clinical protocols.
COMPENSATION
$40–$55 per hour
Company performance-based bonus structure
SCHEDULE
Three 12-hour shifts per week (36 hours)
One weekend shift per month

REQUIREMENTS
Education
Valid NYS Paramedic Certification (required).
High School Diploma or GED (required).
Bachelor's or Associate's degree (preferred).
Experience
Minimum of 3 years of experience in 911, interfacility, critical care, flight, or community/mobile integrated health paramedicine.
Certifications
BCLS
ACLS
NYS Paramedic Certification
Valid NYS Driver's License
Physical Demands
Daily use of personal protective equipment (PPE) and electronic documentation systems.
Frequent and prolonged standing, walking, and patient/equipment transfers.
Prolonged driving between patient visits across the service territory.
Ability to carry a 30–40 lb equipment backpack during patient visits.
Ability to lift up to 40 lbs.
