This role partners closely with medical/psychiatric providers to deliver integrated, patient-centered behavioral health services directly in patients' homes, assisted living facilities, and other community settings.This position is ideal for clinicians who are highly independent, organized, and comfortable working with medically complex older adults. The counselor is responsible for managing their own schedule, providing in-person counseling services, and ensuring all documentation meets Medicare and regulatory compliance standards.Providers are independent contractors.
