We are seeking an experienced Physician Assistant or Nurse Practitioner to join our growing Mobile Primary Care team serving seniors in home-based care, assisted living (ALF), skilled nursing facilities (SNF), and senior living communities.

This is not a traditional clinic role. This is community-based, field-based medicine focused on geriatrics, chronic disease management, dementia care, and value-based care delivery.

If you are a Physician Assistant or Nurse Practitioner passionate about senior care, panel management, and independent practice, this role offers autonomy with strong multidisciplinary support.

What Youll Do

Deliver Mobile Primary Care in home and senior living settings

Provide comprehensive geriatric assessments and chronic disease management

Manage hypertension, diabetes, CAD, CVA, dementia, and complex Medicare populations

Coordinate care with interdisciplinary teams across hospice, palliative, and behavioral health

Drive patient engagement, retention, and outcomes within a value-based care framework

Practice independently while maintaining high-quality documentation and follow-through

What Were Looking For

Active, unrestricted Physician Assistant (PA-C) license

1-2+ years of clinical experience in primary care or geriatrics

Experience in home-based care, SNF, ALF, or senior living communities preferred

Strong clinical judgment and comfort practicing independently

Interest in leadership, panel growth, and clinical program development

Valid drivers license (field-based/mobile role)
