- Consumers' behavioral and physical signs and symptoms are observed and recorded through regular in-home assessments.
- Assistance in maintaining adherence to both physical health and psychiatric medications is provided.
- Medication Management plans are formulated with the participation of the consumer and signed by the consumer and the Community Guidance Center staff.
- Document in the consumer's electronic health record.
- To increase each consumers knowledge of their medication's benefits and side effectsthrough a program of on-going in-home instruction.
- To assist each consumer in creating an environment in which medication adherence is supported by enlisting natural supports and increasing motivational and organizational skills
- Creating lists of medications and allergies to be kept in the consumer' s wallet or purse,
- Suggesting ways to coordinate lab work, and implementing a system for discarding old prescriptions to enhance safety.
- To enhance each consumer's ability to navigate systems of care affecting their medication adherence such as Primary Care Physicians (PCP), labs, specialists, and pharmacies.
- Adhere to seven recognized Sanctuary commitments which includes social responsibility, social learning, non-violence, open communication, democracy, emotional intelligence, growth and change.
