POSITION SUMMARY

The mobile van and School Health Coordinator will perform the coordination of activities that will ensure a solid link between TCA Health's Mobile Health Clinic and the surrounding schools and programs in the community for student and adult access to primary healthcare, early medical intervention and treatment.

ESSENTIAL DUTIES AND RESPONSIBILITIES(Include but not limited to the following):

Serve as a Liaison between school administrators and the Mobile Van for coordination of schedules, monitor the quality of services and service delivery.
Ensure that proper consent forms and authorizations are obtained prior to providing healthcare services on the Mobile Van.
Maintain medical records, collect, and enter data, and greet students.
Ensure Providers are certified with Chicago Public Schools and have process necessary background checks, fingerprints, etc.
Develop strong working relationships with school administrators in service areas and occasionally attend meetings.
Follow-up and coordinate with the Mobile Health Clinic's Mid-Level Provider, School and Medical Assistant regularly to ensure continuous scheduling.
Assist and complete registration and consent forms as needed
Make sure Mobile Van is stocked with all necessary supplies.
Make sure consent forms are obtained before school visits.
Must be trained in the EMR to enter the forms. Everything must be entered before the patient is seen.
Must be trained in the front desk registration process and how to correctly code the students who visit the mobile van. - Physicals and Immunizations
Must protect client confidentiality in compliance with HIPPA.
Prepare reports for all assignments on monthly basis for the Grantors.
Maintain program data and metrics.
Prepare and submit reports as required to the Director of Clinical Services.
Provide accurate, timely information to key stakeholders.
Coordinates visits from the school to the clinic.
Attend meetings (in-person and remote) as scheduled.
Other duties as assigned by management.

REQUIRED SKILLS AND ABILITIES

Successful completion of patient services training.
Successful completion of EPIC training.
Ability to work independently.
Ability to work with individuals who may have a variety of social, economic and health issues.
Ability to form and maintain successful partnerships
Must have a motor vehicle for transportation, a valid Driver' s License and valid car insurance.

QUALIFICATIONS

Able to initiate and maintain working relationships.
Have understanding and knowledge of population and community of service area.
Able to manage time and set priorities.
Good communication skills; able to listen well, use language appropriately, and good writing skills.
Exhibits excellent guest relations with patients, customers, visitors and staff effectively.
Proficient in Microsoft Office (Outlook, Word, Excel)

EDUCATION AND EXPERIENCE

High School diploma or GED equivalent and a minimum of two years of scheduling, patient services, and administration experience or training.
Medical Assistant training preferred.

WORKING CONDITIONS

The average workday will consist of working on a mobile van which may at times be noisy and slightly cramped. Due to weather conditions, your workspace may be inside of different non- traditional settings (school, church, etc.). You will use your personal car to travel to events. Occasionally you will work in a normal office and health center environment.
