The Youth & Family Tree provides services to at-risk youth, ages 12-24, who are experiencing substance abuse and/or generational trauma. This program extends individual trauma treatment to parents and caregivers of the child. The MA/MS Licensed Therapist will develop comprehensive biopsychosocial assessments clinically responsive to age-appropriate patient/family needs and concerns commensurate with knowledge in the substance abuse/mental health field.
Key Responsibilities:
Collaborate with other professionals to evaluate consumers' medical or physical condition and to assess needs.
Provide documentation of clinical and case management services in accordance with department standards.
Participate with the interdisciplinary team in developing and implementing clinical treatment plans for consumers. Serve as consumer advocate for needs within the community.
