Key Responsibilities
- Assess and evaluate members'needs and requirements to support optimal health and wellness outcomes.
- Guide members and families toward appropriate healthcare, social service, and community resources.
- Employ a variety of strategies, approaches, and techniques to address members'health and wellness needs.
- Identify and resolve barriers that may hinder effective care.
- Monitor members'progress toward desired outcomes through assessments, data review, conversations with members, and active care planning.
- Facilitate interaction between members and appropriate healthcare or community resources.
- Maintain accurate and timely documentation using electronic case management and other computer systems.
- Follow established professional standards, regulations, procedures, and organizational guidelines.
- Make decisions regarding work priorities and approach while following established direction and procedures.
- Support an organization focused on continuously improving the consumer experience.
Required Qualifications
- Bachelor's degree.
- 2+ years of related experience.
- Proficiency with Microsoft Office, including Word and Excel.
- Exceptional verbal, written, and interpersonal communication skills.
- Ability to use a variety of electronic information applications and software programs.
- Passion for contributing to an organization focused on continuously improving consumer experiences.
- Valid state driver's license.
- Proof of personal vehicle liability insurance with minimum coverage limits of $100, 000/$300, 000/$100, 000.
- Ability and willingness to travel throughout Pasco County.
- This position is patient-facing and requires participation in the organization's Tuberculosis (TB) screening program. Selected candidates will be required to complete TB screening.
Preferred Qualifications
- Master's degree.
- Prior experience working with Medicare and Medicaid recipients.
- Previous experience with electronic case-note documentation.
- Experience documenting information across multiple computer applications and systems.
- Experience in health promotion, coaching, and wellness.
- Knowledge of community health and social service agencies.
- Familiarity with local and additional community resources.
- Previous experience as a Care Coach for *** within the past few years.
