Type: Travel Contract | 13 Weeks

Location: Providence Health & Services Laguna Facility (Floating Required)

Schedule: Day Shift | 5x8 Hours | 8:00 AM 4:30 PM | Variable 5-day workweek (Department operates 7 days/week. Weekend coverage required as scheduled. Days off vary and are not guaranteed to be consecutive.)

Job Description Summary:

Seeking an experienced Medical Social Worker (MSW) for an acute care inpatient hospital setting within Providence Health & Services. This is a travel assignment requiring recent inpatient hospital experience, strong discharge planning and case management skills, and proficiency with Epic EMR. The social worker will manage a caseload of approximately 20 23 patients while collaborating with multidisciplinary teams to coordinate safe and effective discharge planning and continuum of care.

Responsibilities:

• Conduct comprehensive biopsychosocial assessments.
• Develop and implement individualized plans of care.
• Coordinate patient discharge planning and transition of care.
• Perform inpatient case management and utilization review.
• Coordinate referrals to rehabilitation, long-term care, home health, DME, and community resources.
• Collaborate with physicians, nursing staff, case managers, therapists, and outside agencies.
• Educate patients and families regarding available community resources and benefits.
• Provide crisis intervention and psychosocial support.
• Assist with advance directives, guardianship, power of attorney, and protective services referrals when appropriate.
• Document all assessments, interventions, discharge planning, and patient education accurately in Epic.
• Participate in interdisciplinary care conferences and care coordination meetings.
• Maintain compliance with hospital, state, and NASW standards.

Key Requirements:

• Recent acute care inpatient hospital experience is required.
• Minimum 2 years of experience (recent hospital experience strongly preferred; must have acute experience within the last few months).
• No first-time travelers (Hard Stop).
• Travel experience is required.
• Epic EMR experience is a Hard Stop.
• Experience with discharge planning, care coordination, utilization review, and inpatient case management.
• Expected caseload of approximately 20 23 patients.
• Floating to Laguna Facility required.
• Weekend rotation required as department operates 7 days per week.
• Maximum 7 days RTO accepted.
• Pending license accepted but must be active by the start date.

Qualifications Education:

• Master's Degree in Social Work (MSW) from an accredited program.

Experience:

• Minimum 2 years of Medical Social Work experience.
• Recent acute care inpatient hospital experience required.
• Previous travel assignment experience required.
• Experience in discharge planning, inpatient case management, care coordination, crisis intervention, and utilization review.
• Trauma Level I or II experience is preferred but not required.

License:

• Active MSW required.
• LCSW preferred.
• BLS certification required.
• Pending state license accepted if active prior to start date.
