Social Worker –Community Guardian Program

$25–$29/hour DOE
Full-Time | Monday–Friday | 9: 00 AM–5: 00 PM

Make a meaningful impact supporting some of New York City’s most vulnerable adults. We’re seeking a compassionate and organized Social Worker to provide case management, advocacy, referrals, and community-based support to clients throughout the five boroughs.
What You’ll Do

• 
  Manage and monitor an assigned client caseload
  
• 
  Assess client needs, safety, functioning, and eligibility for services
  
• 
  Advocate for clients and develop individualized service plans
  
• 
  Coordinate referrals for Medicaid, Medicare, SNAP, home health aides, public assistance, legal aid, medical/psychiatric evaluations, protective services, and more
  
• 
  Conduct home, facility, and community visits throughout NYC
  
• 
  Attend court hearings and community meetings
  
• 
  Maintain case records and documentation using HRA and agency databases
  
• 
  Collaborate with community agencies, healthcare providers, legal teams, and other professionals
  

Qualifications

• 
  Master’s degree in Social Work, Counseling, Psychology, Gerontology, or a related field
  
• 
  Strong writing, documentation, organizational, and time-management skills
  
• 
  Computer proficiency
  
• 
  Ability to travel throughout all 5 boroughs
  
• 
  No license required
  

$25–$29/hour DOE | Full-Time

The official JD also notes that certain emergency situations may occasionally require work outside the normal workweek, including evenings, weekends, or holidays.
