JOB SUMMARY

The Volunteer Coordinator is responsible for recruiting, training, and matching volunteers with hospice patients based on patient preferences/ needs. The medical social worker contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement.

DUTIES & RESPONSIBILITIES
  • Recruits, interviews, evaluates, hires and trains volunteers for the hospice.
  • Conducts volunteer orientation per Agape Hospice policies.
  • Maintains HR files for all volunteers to remain compliant with requirements.
  • Ensures that volunteers understand their role in the hospice process and plan of care.
  • Assigns and schedules volunteers in the day-to-day administrative and/or direct patient care roles that best match their capabilities.
  • Maintains records of volunteers' activities and keeps patient charts updated to reflect volunteer activity.
  • Actively participates in the interdisciplinary group meeting and documents appropriately.
  • Participate in Veteran Honoring's for veteran patients of Agape who accept.
  • Ensure that volunteer hours meet or exceed 5% of total patient care hours.
  • Monitor and document the amount of cost savings achieved through use of volunteer hours.
  • Conduct ongoing education, training and appreciation activities directed to the volunteers
  • Be the liaison for the We Honor Veterans program
  • Serves as liaison between patients and families/caregivers and community agencies
  • Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required.
  • Carries out social evaluations and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties.
  • Maintains clinical records on all patients referred to social work
  • Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
  • Provides information to patients and families/caregivers and community agencies.
  • Participates in the development of the individualized plan of care and attends regularly scheduled interdisciplinary group meetings
  • Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.
  • Actively participates in quality assessment performance improvement teams and activities.

JOB REQUIREMENTS (Education, Experience, Knowledge, Skills & Abilities)
  • A graduate of a bachelor's program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in 418.114(b)(3).
  • At least three years of experience in health care, preferably in hospice or home health.
  • Proficiency in computer applications to include MS Word, Excel and Outlook.
  • Excellent customer service and communication skills.
  • Knowledge of CMS regulations and federal/ state regulations.
  • Understanding of HIPAA guidelines and maintaining confidentiality of patient information.
  • Ability to work independently and respond well to changing priorities and conditions.
  • Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
  • Valid Arizona Driver's license, proof of insurance, and a reliable vehicle.
