Multi-Site MDS Coordinator (RN)

Full-Time | Primarily Remote with Travel 1-2 Days Per Week

Salary: $115,000 Annually

We are seeking an experienced MDS Coordinator (RN) to support skilled nursing facilities in Andover, North Andover, Ayer, and Agawam, Massachusetts. This full-time position is primarily remote and includes periodic travel to assigned facilities as needed. Travel is typically limited to approximately 1-2 days per week.

The ideal candidate is detail-oriented, organized, and comfortable working independently while collaborating with interdisciplinary teams to ensure accurate assessments, regulatory compliance, and quality resident care.

Responsibilities

• Complete and submit MDS assessments accurately and within required timeframes
• Conduct resident interviews and gather supporting assessment information
• Develop, review, and update resident care plans
• Participate in care plan and interdisciplinary team meetings
• Review assessments for accuracy, compliance, and reimbursement optimization
• Support PDPM, CMI, Quality Measures (QM), and QRP initiatives
• Provide MDS coverage during staffing transitions, leaves, and open positions
• Collaborate with facility leadership and clinical teams to support quality outcomes

Qualifications

• Active RN license in Massachusetts or eligibility for Massachusetts licensure
• Minimum 2 years of MDS experience required
• Skilled nursing or long-term care experience required
• Knowledge of MDS 3.0, PDPM, CMI, QM, and QRP preferred
• Strong communication, organizational, and interpersonal skills
• Computer proficiency and experience with EMR systems
• Ability to travel to assigned facilities as needed
