We are currently looking for a Licensed Clinical Social Worker LCSW to join our physician-owned nephrology practice in Reston, VA. Outstanding opportunity to join a highly regarded private practice led by a multi-award-winning physician consistently recognized as a Washingtonian Top Doctor.

Work alongside our nephrologist nurse practitioner and renal dietitian to provide holistic care, helping educate patients on chronic kidney disease and manage end-stage renal disease. Delivering essential psychosocial support and care coordination in an outpatient setting.

Key Focus Areas: • Assisting patients with insurance transitions • Medicare benefit counseling • Medication burden management

• Providing case management • Discharge planning • Psychotherapy • Psychoassessments • Life coaching • Transition of care

• Conducting home and virtual visits • Teaching through our CKD education program

We prioritize work-life balance. Flexible part-time schedule, 2-3 days per week. No weekends, evenings, or on-call requirements.

Qualifications: Education: Masters degree in Social Work required. Licensure: Licensed Clinical Social Worker LCSW in the state of Virginia is required.

Experience: Previous experience in nephrology or dialysis social work is preferred, but new graduates are encouraged to apply.

Must love building relationships with patients, value bedside manner, and possess excellent clinical skills. Must comply with HIPAA rules and regulations.
