Working as a member of the clinic-based care team, the MTM Pharmacist proactively and collaboratively identifies and coordinates the resolution of medication-related issues based on an interdisciplinary, patient-specific care plan. Working at the top of their license and taking a comprehensive approach, the pharmacist helps patients achieve their best possible outcomes from their medication therapies. The pharmacist also serves as the medication expert to raise the overall knowledge of the care team and improve workflows where possible.
Strong preference for candidates residing within the UPH geographies of Iowa, Illinois, & Wisconsin.
