Base Pay: $25.00-$29.55 per hour (based on experience)

Shift: 11:00 PM-7:00 AM

Glen Haven Health & Rehabilitation is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our skilled nursing and rehabilitation team on the 3:00 PM-11:00 PM evening shift.


As a n Evening Shift LPN, you'll provide exceptional nursing care to residents in our long-term care and rehabilitation community while working alongside an experienced interdisciplinary team dedicated to delivering quality, resident-centered care.


Licensed Practical Nurse (LPN) Responsibilities

  • Provide direct nursing care to residents in a skilled nursing and long-term care setting.
  • Assess, monitor, and document changes in resident conditions.
  • Administer medications and treatments according to physician orders.
  • Monitor resident response to medications and treatments.
  • Collaborate with physicians, Registered Nurses, therapists, and other healthcare professionals to develop and implement individualized care plans.
  • Accurately complete electronic nursing documentation.
  • Respond to changes in resident condition and communicate concerns promptly.
  • Supervise and support Certified Nursing Assistants (CNAs) to ensure quality resident care.
  • Maintain compliance with state and federal regulations and facility policies.

Qualifications

  • Current Licensed Practical Nurse (LPN) license in good standing.
  • Current CPR/BLS certification.
  • Experience in skilled nursing, rehabilitation, long-term care, post-acute care, or geriatric nursing is preferred but not required.
  • Strong clinical assessment, communication, and organizational skills.
  • Ability to work independently while collaborating with the interdisciplinary healthcare team.
  • Compassionate, dependable, and committed to providing outstanding resident care.
