Shift Details:

• 
  3 days/wk - M-F (any three days ok, but must be consistent)

Nurse Practitioner (NP)  Duties & Responsibilities:

• 
  Perform comprehensive patient assessments, examinations, and medical histories.
  
• 
  Diagnose and treat acute and chronic health conditions.
  
• 
  Order, interpret, and evaluate diagnostic tests and laboratory results.
  
• 
  Prescribe medications and develop individualized treatment plans within scope of practice.
  
• 
  Educate patients and families on disease prevention, treatment options, and health maintenance.
  
• 
  Collaborate with physicians and interdisciplinary healthcare teams to coordinate patient care.
  
• 
  Monitor patient progress and adjust treatment plans as needed.
  
• 
  Document patient encounters accurately in the electronic medical record (EMR).
  
• 
  Promote preventive care and wellness through patient education and screenings.
  
• 
  Ensure compliance with clinical guidelines, facility policies, and regulatory standards
  

Minimum Qualifications 

• 
  Current, valid, and unrestricted WI license/certification in good standing, as required for the position.
  
• 
  Minimum of 1+ year within the last 3 years recent experience as an NP
  
• 
  IP/FP experience needed.
  
• 
  Population served is adult male - older with multiple comorbidities.
  
• 
  Long term care experience could also be helpful.
  
• 
  Corrections experience ideal, but not required.
  
• 
  Not seeking new grads 
  
• 
  Copy of current, completed Primary Source Verification (PSV) from the appropriate licensing board or verification agency, if required.
  
• 
  Copy of all current required certifications and compliance documents (e.g., .BLS, N95 fit test, immunization records, TB test, or other client-specific requirements).
