What You’ll Do

As an LPN with UChange Home Care Services, you will provide skilled nursing services in accordance with the patient's individualized plan of care, physician orders, applicable regulations, and agency policies.

Responsibilities may include:

• Providing skilled nursing care in the client's home
• Assessing and monitoring the patient's condition within the LPN's scope of practice
• Following physician orders and the established plan of care
• Administering medications and treatments as ordered and within the LPN's scope of practice
• Monitoring vital signs and documenting patient observations
• Identifying changes in the patient's condition and promptly reporting concerns to the appropriate healthcare professional
• Providing wound care and other skilled nursing interventions when ordered and within scope of practice
• Assisting with patient education and reinforcing instructions provided by the healthcare team
• Communicating with physicians, registered nurses, supervisors, patients, and family members as appropriate
• Maintaining accurate and timely clinical documentation
• Completing required visit notes and electronic documentation
• Following applicable EVV, documentation, infection-control, and compliance requirements
• Maintaining patient confidentiality and protecting protected health information
• Participating in care coordination and communication with the interdisciplinary healthcare team
• Providing compassionate and respectful care that supports the patient's safety, comfort, and independence

What We’re Looking For

We are seeking LPNs who demonstrate strong clinical judgment, professionalism, accountability, and compassion.

Qualifications include:

• Current and unrestricted Illinois LPN license
• Graduate of an approved practical nursing program
• Previous home health experience preferred
• Strong clinical assessment and documentation skills
• Ability to work independently and manage responsibilities in a home-based setting
• Excellent communication and interpersonal skills
• Ability to follow physician orders and individualized plans of care
• Reliable attendance and punctuality
• Reliable transportation when required for patient assignments
• Valid driver's license and automobile insurance when driving is required
• Reliable smartphone or mobile device for communication and electronic documentation
• Ability to successfully complete required background checks and other applicable pre-employment requirements
• Authorization to work in the United States
