Join Optum's Senior Community Care team and provide meaningful, longitudinal care to medically complex patients in skilled nursing facilities throughout Multnomah, Yamhill and Marion Counties.

As the primary Advanced Practice Clinician for your patient panel, you make the clinical decisions while being supported by a fully integrated interdisciplinary team. Our value-based model focuses on quality of care, not patient volume or productivity quotas.

This is a per diem, field-based position providing longitudinal care to patients in various skilled nursing facilities.

Make an Impact with Complex Patient Care

As part of Senior Community Care, you'll care for some of Optum's highest-risk Medicare members, including patients with multiple co-morbidities, advanced disease states and complex treatment needs.

You'll address the whole patient, including medical, behavioral, functional and social needs, while partnering with caregivers, facility teams and other healthcare professionals to improve outcomes and quality of life.

Primary Responsibilities

  • Conduct comprehensive patient assessments
  • Manage acute and chronic conditions, including testing, diagnostics and interventions
  • Manage members during a short term sub-acute rehabilitation episode
  • Develop and manage complex treatment plans
  • Lead advanced illness, goals-of-care and advance care planning conversations
  • Collaborate with the interdisciplinary care team, facilities and caregivers
  • Provide patient and caregiver education
  • Ensure accurate and complete clinical documentation

Required Qualifications:

  • For NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care

  • For PAs: Current NCCPA certification and state licensure

  • Active, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations

  • Active and unrestricted driver's license

  • In addition to a driver's license, access to reliable transportation

  • Current, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)

  • Ability to gain a collaborative practice agreement, if applicable in your state

  • Ability to navigate varied environments and to position oneself as needed to perform job duties

Preferred Qualifications:

  • 1+ years of clinical experience as an APC (long-term care setting preferred)
  • Experience in geriatric medicine, long-term care, senior living or home care setting
  • Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Understanding of Geriatrics and Chronic Illness
  • Understanding of Advanced Illness and end of life discussions
  • Proficiency with electronic medical records and technology

Compensation for this specialty generally ranges from $53.00 to $79.00 per hour. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
