JOB REQUIREMENTS- Board: BC required (or BE)
- Licenses: Licensed in OR
- DEA: DEA required
- Certifications: BLS
- Experience: Minimum 1-2 years geriatric experience preferred
- Other Qualifications: Epic EMR proficiency, team-based care experience
Duties & Responsibilities- Provide comprehensive primary care to elderly patients in clinic and community settings.
- Perform home visits to assisted living, SNF, and personal residences.
- Collaborate with interdisciplinary team (11 members) to coordinate patient care.
- Manage patient care via Epic EMR.
- Participate in PACE program, focusing on holistic care for over-55 population.
