• Reason For Need -FMLA
• Will you accept Multiple providers Yes
• Assignment Start & End date -ASAP to 3/30/2023
• Schedule Hours for Clinic /Hospital 40 hours
• Schedule Days Clinic /Hospital Monday-Friday
• Number of patients seen per day in hospital 1 to 10 on average
• Call Night /Weekend - 1 weeknight each week, weekend call rotates every 4th and 5th weekend
• What are the days for Call- 1 weeknight each week, weekend call rotates every 4th and 5th weekend
• Call will be for assigned, unassigned, or both assigned and unassigned patients Assigned
• Call response time required? (15 min? 20 min?) 30 minutes
• Average Phone consults while on call 0
• Average admissions while on call- 1 to 5
• Average patients seen while on call- 1 to 5
