Job Title: Nurse Care Manager

Work Schedule: Monday - Friday, 8 a.m. - 5 p.m.
Summary
The Nurse Care Manager serves as the dedicated clinical care coordinator for the Baylor Longevity Health Program, partnering closely with the Executive Medical Director to deliver highly personalized, proactive care management for a defined panel of longevity health members. This role coordinates advanced diagnostics, preventive care planning, specialty collaboration, and ongoing health monitoring to support optimal health outcomes and longevity. The Nurse Care Manager acts as the primary clinical point of contact for members, ensuring seamless care coordination, timely communication, and a high-touch, concierge-level patient experience. The Nurse Care Manager supports continuity of care across all services, acting as an advocate for members and facilitating access to appropriate resources to optimize overall health outcomes. It does not manage staff.
This role supports a premium concierge-style longevity medicine program designed to deliver highly personalized, proactive care focused on optimizing long-term health outcomes. The Nurse Care Manager will work closely with executive leadership, physicians, and interdisciplinary care teams to coordinate advanced diagnostics, specialty consultations, and individualized care planning for a defined member panel. The position requires strong clinical judgment, exceptional communication skills, and the ability to manage complex care coordination in a high-touch, patient-centered environment.
This position is located at our McNair Campus.
Job Duties
Provides patient and family education regarding preventive care strategies, risk reduction, and disease management to support long-term health optimization and adherence to individualized care plans.
Supports development and implementation of individualized longevity health plans in partnership with the Executive Medical Director and program leadership.
