Job Requirements:
Degree/Education: Associate Degree of Nursing, Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE). Current, full, active, and unrestricted license to practice as a Registered Nurse.
Experience:Possess one of the following certifications: Commission for Case Manager Certification Certified Case Manager (CCM) Certification of Disability Management Specialists Commission: Certified Disability Management Specialist (CDMS), Association of Rehabilitation Nurses: Certified Rehabilitation Registered Nurse (CRRN), American Board for Occupational Health Nurses Certified Occupational Health Nurse (COHN) or Certified Occupational Health Nurse-Specialist(COHN-S). National Board for Certification in Continuity of Care: Advanced Certification in Continuity of Care (ACCC), Commission on Rehabilitation Counselor Certification: Certified Rehabilitation Counselor (CRC) American Nurses Credentialing Center Nurse Case Manager (RN-NCM), National Academy of Certified Care Managers: Care Manager Certified (CMC) A minimum of 3 years (ADN) or 2 years (BSN) full-time experience in clinical case management with diverse populations. Familiarity with the military healthcare system and telehealth experience is desirable. Work experience with Tri-care and Military Health System Genesis system preferred.

Comprehensive Case Management: Provide expert case management for military members, veterans, and their families across various healthcare settings, focusing on acute, chronic, and complex health issues.
Patient Advocacy: Serve as a dedicated advocate for military members, ensuring they receive the highest standard of care, while addressing their physical, emotional, and psychosocial needs.
Collaborative Care Coordination: Work seamlessly with interdisciplinary healthcare teams to coordinate and integrate care across all levels and sites, facilitating a holistic approach to patient well-being.
Military Community Engagement: Engage with the military community, fostering relationships with service members and their families, and contributing to the unique healthcare challenges faced by those serving in the armed forces.
Innovative Healthcare Solutions: Contribute to the development and implementation of innovative healthcare solutions, including telehealth initiatives, to enhance accessibility and quality of care for military members.
