The Nurse Case Manager will work with patients, families, physicians, and staff to help coordinate safe and effective care. Candidates must be available for assigned shifts, including overnight shifts, with coverage across all seven days of the week. The department’s shifts are bid on by seniority. Position Overview Fusion HCR is seeking a Nurse Case Manager to assess, plan, coordinate, and evaluate patient care needs across the healthcare continuum. In this role, you will serve as an advanced clinical resource for patients and families, as well as for physicians and hospital staff. You will contribute to a coordinated, multidisciplinary approach to care, helping support effective care delivery for patients with a range of conditions and acuity levels. The position calls for strong clinical judgment, the ability to interpret medical records and documentation, and clear, compassionate communication in high-stress situations. You will also apply case management principles and relevant regulatory and reimbursement guidelines in supporting patient care. Availability to work assigned shifts—including overnight shifts—and provide coverage seven days a week is required. Key Responsibilities
- Assess patient needs and contribute to care planning, coordination, and evaluation across the healthcare continuum.
- Serve as a clinical resource for patients, families, physicians, and hospital staff.
- Coordinate with interdisciplinary team members to support safe and effective care delivery.
- Review and interpret medical records and clinical documentation to inform case management activities.
- Apply case management principles, standards of care, and relevant reimbursement and regulatory guidelines.
- Support discharge planning and utilization review activities as needed.
- Communicate clearly and compassionately with patients and families from diverse backgrounds, including in high-stress situations.
- Follow hospital safety practices, patient rights requirements, infection control policies, and emergency procedures.
- Work assigned shifts, including overnight shifts, and be available for coverage seven days a week.
Required Qualifications
- Graduation from an accredited school of nursing.
- At least three years of nursing experience in an acute care hospital setting. Experience in a psychiatric hospital does not meet this acute care requirement.
- At least one year of experience in case management, discharge planning, or utilization review.
- An active Nevada Registered Nurse (RN) license.
- Basic Life Support (BLS) certification from the American Heart Association.
- A master’s degree in nursing with a concentration in case management may substitute for the required experience at the employer’s discretion.
- Ability to manage patients with varying conditions and acuity levels, interpret clinical records, use healthcare software, and apply regulatory and reimbursement guidelines.
- Strong communication and collaboration skills, along with the ability to use equipment safely and effectively.
- Availability to work assigned shifts, including overnight shifts, across a seven-day schedule.
- Preferred: Three or more years of acute care case management experience, recent acute care hospital experience, and pediatric case management experience. Recent Emergency Department or Utilization Review experience is a plus. CCM or ACM certification, or membership in CCMC, ACCM, or ACMA, is strongly preferred.
