Graduate from an accredited nursing program (ADN or BSN)
Current, active, unrestricted Registered Nurse (RN) license in any U.S. state or territory
Current Basic Life Support (BLS) certification
Ability to obtain a Case Manager certification within 12 months of hire and maintain certification throughout employment
Minimum of 2 years of recent clinical competency in the applicable nursing specialty (within the last 2 years)
Demonstrated experience in case management, including patient assessment, care coordination, and treatment planning
U.S. Citizenship required
Participate in all phases of the Case Management Program (CMP) to ensure compliance with established case management standards of care
Perform comprehensive case management functions including patient assessment, care planning, implementation, coordination, and ongoing monitoring
Identify patients appropriate for case management services and integrate local case management processes
Develop and implement case management strategies across inpatient, outpatient, on-site, and telephonic settings
Create and utilize tools to support case management, including patient identification methods, assessment tools, clinical practice guidelines, and community resource databases
Coordinate care across interdisciplinary teams, integrating nursing case management with social work and utilization management functions
Maintain liaison with community agencies and external organizations to support continuity of care
Develop individualized treatment plans incorporating preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions
Ensure proper execution of treatment plans and coordinate follow-up care in ambulatory and community healthcare settings
Accurately collect, track, and document patient care data in accordance with MTF policies and standards
Provide patient and caregiver education based on identified needs to support treatment adherence and health outcomes
