General Description
The Nurse Case Manager is responsible for working with Service Providers within and outside of the Health Department to facilitate patients access to care in accordance to Illinois Breast and Cervical Cancer Program (IBCCP) guidelines. Coordinates and assist Assistant Nurse Coordinator to provide education, information, and access to human services available in the community.
Scheduled Hours: 37.5 hours per week

Knowledge Skills Abilities and Education Required
Illinois licensure as a Registered Professional Nurse or Illinois licensure by reciprocity within six months
Requires a Bachelor degree in Nursing, an Associate Degree in Nursing or graduation from a diploma School of Nursing.
With a Bachelors degree, one year of clinical case management or two years of work in a health-related field.
With an Associates degree, two or more years of clinical case management experience.
