Description: The Nurse Case Manager is responsible for working with Service Providers within and outside of the Health Department to facilitate patients access to care in accordance to Illinois Breast and Cervical Cancer Program (IBCCP) guidelines. Coordinates and assist Assistant Nurse Coordinator to provide education, information, and access to human services available in the community.
Scheduled Hours: 37.5 hours per week Monday to Friday (9:00AM - 5:00PM)
Essential Functions: • Works as a liaison between LCHD/CHC and outside providers of care. • Assists in coordination of Primary Care services with other Health Department programs and community agencies. • Participates in identifying the client needs, makes appropriate referrals. • Follows up on referrals of IBCCP clients, tracking and monitoring test results by contacting the hospitals or the providers. • Provides clients with health education, laboratory and consultation results and recommendations and explanations in a language appropriate to the client's level of understanding. • Maintains client records, documenting interactions, services provided, phone calls. • Participates in CQI activities to assess and monitor client care processes and develop plans for improvement when necessary. • Demonstrates critical thinking skills in collaboration with others as appropriate. • Exhibits a sense of ownership in decision-making activities. • Attends staff meetings and team conferences. • Attends in-service education, staff meetings and workshops that are appropriate for professional development. • Educates the clinic staff about services of IBCCP, providing in-services at staff meetings as needed. • Maintains orderliness and cleanliness of work area. • Performs other duties as required or assigned.
Knowledge, Skills, Abilities, and Education Required: Illinois licensure as a Registered Professional Nurse or Illinois licensure by reciprocity within six months Requires a Bachelor degree in Nursing, an Associate Degree in Nursing or graduation from a diploma School of Nursing. With a Bachelors degree, one year of clinical case management or two years of work in a health-related field. With an Associates degree, two or more years of clinical case management experience.
