Case Manager
Pay Rate: $55–$58/hr
Schedule: Monday–Friday, 8:30 AM–4:30 PM
Location: Summit, NJ
Setting: Acute Care Hospital
Education: BSN required
License: Active RN license required
Certification: CCMC preferred
Job Responsibilities
Responsibilities
- Coordinate patients throughout the acute care hospitalization.
- Manage all aspects of the discharge planning and transition process.
- Perform utilization management and quality screenings.
- Handle inpatient case management responsibilities.
- Coordinate care and services to support appropriate patient outcomes and transitions.

Qualifications
Required Qualifications
- Inpatient Case Management experience is required.
- RN license required.
- BSN required.
- 2 years of acute care experience strongly preferred.
- Experience in case management and managed care.
- Strong understanding of discharge planning and transitions of care.
Preferred Qualifications
- CCMC certification.
- 2+ years of acute-care experience.
- Experience with utilization management.
- Experience performing quality screenings.
