The Manager has overall day-to-day responsibility for directing and managing the activities of the Case Management Team. Facilitates the Hospital’s goals of reducing length of stay, improving patient care, efficient and effective utilization of resources to ensure appropriate continuum of care of patients.

 

 

NURSE CASE MANAGER (NYSNA)

CASE MANAGEMENT

9am-5pm / Alternating Weekends / Full-Time

Responsibilities:
  • Professionally direct activities of support persons, volunteers, students, Community Senior Aides and others.
  • Participate in studies, surveys, or research, initiated or approved by the Case Management Department to develop knowledge about effective treatment, or improve quality of delivery of service/care.
  • Identify quality of care issues and refer to appropriate departments/ services.
  • Participate in conferences, workshops, and other professional development activities to maintain licensure and/or remain professionally current with advances in the field of expertise.
  • Participate in multidisciplinary task forces, committees, and projects.
Qualifications:

Education:

BSN preferred. 

Degree in Nursing or an allied health field, or a suitable combination of education and experience required.

Current NYS licensure as RN required.

 

Experience:

Minimum of 4 years of direct experience in utilization management, discharge planning, case management or home care management required.

 

Skills:

Current clinical and technical nursing skills.

Knowledge of rules and regulations of child abuse/neglect reporting as appropriate.

Familiarity with requirements of regulatory agencies and third party payors.

Demonstrated ability to use word processing, spreadsheet and/or database programs as required by the position.

Excellent oral and written communication skills.

Excellent interpersonal skills.

Good problem-solving, decision-making and judgment skills.

Must read, write and speak English to the extent required by position; knowledge of second language preferred and may be required for some positions.

JOB SETTING/PHYSICAL DEMANDS

 

Patient care areas in-patient/out-patient throughout the medical center; contact with patients, families, significant others, visitors and staff.

Mainly sedentary with occasional standing or walking.

May be required to make home visits.

24-hour, 7 day/week on-call availability required.
