1st Call Home Healthcare is hiring a full time RN/LPN Homecare Nurse Coordinator to cover our West Michigan Territory. Must love to drive coverage area includes mostly Grand Rapids, with some Battle Creek and Kalamazoo areas. The Nurse Coordinator assumes responsibility and accountability for the delivery of industry recognized home care nursing and health related services to the agency’s patients Duties Include: Perform start of care visits and establish care plans. Monthly follow up visits.
