Nature of Position:

The Nurse Delegate must be able to work in a Patient Centered Medical Home model. Must facilitate partnerships between patients, physicians and health teams with focus on care coordination and integration of treatment internally and externally. Must work diligently to assure that services are accessible, continuous, comprehensive, coordinated, compassionate, and culturally effective. Must be committed to eliminating barriers to care that is centered on the needs and convenience of patients above all other factors. The Nurse Delegate supports medical providers. By managing provider task boxes, and in accordance with Neighborhood Health protocol, the Nurse Delegate autonomously responds to tasks, relays results/clinical information to patients, follows straight-forward clinical protocols for diseases and conditions and schedules appointments when needed.

Summary of Duties:

  • Closely monitor assigned provider task boxes and Provider Approval Queues.
  • Sign off on/closeout tasks that are in the scope of the Nurse Delegate Protocol.
  • Telephonically deliver clinical instruction from the provider to the patient.
  • Telephonically communicate various results (i.e., labs, imaging, etc.) to the patient.
  • Complete public health reporting forms, disability and FMLA paperwork.
  • Exhaust every effort to reach patients to ensure they receive vital information.
  • Schedule patient appointments when follow-up is needed.
  • Promotes clear communication amongst a care team and treating clinicians by ensuring awareness regarding patient care.
  • Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures.
  • Establish trust and rapport with patients.
  • Use clinical thinking and motivational interviewing to keep high-risk/high-utilization patients engaged in care.
  • Must work at least 1 day per week at an on-site clinic.
  • Other duties as assigned.

Education and Experience:

  • Essential:

  • Licensed LPN or higher; RN Preferred

  • Must be licensed to practice in Tennessee

  • Experience in Ambulatory, Physician Practice, or Community Health settings

  • Good basic technical knowledge and skills related to clinical duties

  • Proficient computer skills, including Microsoft Office

  • Ability to deliver clinical instruction and work with people in a diverse population with mixed levels of health literacy.

  • Self-disciplined, energetic, passionate, and innovative.

  • Self-starter with the ability to work in an autonomous role.

  • A team player that can follow a system and protocol to achieve a common goal

  • Highly organized and well-developed oral and written communication skills

  • Demonstrates sound judgment, decision-making and problem-solving skills

  • Able to maintain confidentiality with all aspects of information in accordance with practice, State and Federal regulations

  • Confidence to communicate and outreach to patients.

  • Excellent customer service skills.

  • Preferred:

  • Bilingual in English and Spanish

  • Experience working in the following areas: prenatal, women's health, pediatric, and chronic diseases.

  • Experience with telephonic outreach.

  • 1 year in a Patient-Centered Medical Home setting or knowledge of the Patient-Centered Medical Home initiative

  • Experience with an Electronic Medical Record (NextGen) preferred, and other general office skills.

Salary Grade and Classification: Exempt
