    Position Details:

    • Main Office location: Westwood, MA
    • Service Area: This position requires travel to our multiple locations in New England, Pennsylvania and Virginia.
    • Job Type: Full Time
    • Schedule: Monday - Friday
    • Base Salary Range: $95,000 - $105,000

    Responsibilities:

    • Assist in creating and implementing education curriculum with Clinical Education department.
    • Demonstrates techniques and best practices for trainees.
    • Assists in creating and implementing policies and procedures for all clinicians.
    • Initiate appropriate preventive and rehabilitative nursing procedures.
    • Administer medications and treatments as prescribed by physicians.
    • Conduct initial assessments of patients and families to determine home care needs.
    • Perform thorough physical assessments and gather comprehensive medical histories.
    • Develop and revise individualized plans of care based on patient status and evolving needs.
    • Collaborate closely with physicians, nurse practitioners, and interdisciplinary teams to create robust Plans of Care for each patient.
    • Responsively address inquiries and concerns from patients, caregivers, and medical teams.
    • Provide counseling and support to patients and caregivers before, during, and after treatment.
    • Supervise home health aides as required.
    • Identify discharge planning needs and proactively implement them as part of the care plan development. 

    Qualifications:

    • Preferably 2+ years of nursing experience, with a focus on geriatrics.
    • Nursing degree from an accredited institution.
    • Excellent bedside manners and exceptional communication skills.
    • Working knowledge of healthcare laws and regulations.
    • Maintain current BLS CPR certification.
    • Possess a valid driver's license and own a reliable, insured vehicle.
