Nurse - Hospice

Required
Active Registered Nurse (RN) License – New Jersey
Minimum 2 years of RN experience in a home hospice setting
Experience with EMR systems
Strong clinical judgment, critical thinking, and communication skills
BLS certification
Valid New Jersey driver’s license
Associate’s Degree in Nursing (ASN) or Diploma in Nursing

Preferred
3+ years of RN experience
Prior GIP (General Inpatient) hospice experience in a hospital setting

• Perform comprehensive patient assessments to determine eligibility and appropriateness for hospice care
• Develop, implement, and evaluate individualized plans of care for patients and families
• Provide case management, prioritizing patient needs and coordinating services across the care team
• Deliver age-appropriate education to patients and caregivers; adjust teaching plans as needed
• Maintain accurate and timely documentation, including admissions, revisits, and discharges within required timeframes
• Ensure compliance with hospice regulations, clinical standards, and insurance/reimbursement guidelines
• Monitor patient safety, medication management, and infection control practices
• Collaborate with interdisciplinary team (IDT) and participate in bi-weekly care meetings
• Conduct HHA (Home Health Aide) supervision per standards
• Support performance improvement initiatives and participate in staff meetings and trainings
• Assist with onboarding, mentoring, and cross-training team members as needed
