Join Our Caring Team as a Licensed Practical Nurse!

Are you a compassionate, dedicated Licensed Practical Nurse (LPN) looking to make a meaningful difference in the lives of residents? We invite you to join our team at Evergreen, where we prioritize exceptional resident care in a warm and supportive environment!

This position is 8 shifts in a two week pay period on the PM shift (2:30pm-10:45pm) for a total of 32 hours per week.

What You’ll Do:

• Deliver compassionate, patient-centered care to our residents
• Assist with daily living activities, promoting independence and dignity
• Monitor vital signs and health status, ensuring the best possible outcomes
• Collaborate with a dedicated team of healthcare professionals to create individualized care plans
• Educate residents and their families about health management and wellness

What We’re Looking For:

• A current LPN license in Wisconsin
• A genuine passion for helping others and a strong commitment to resident care
• Excellent communication skills and the ability to work collaboratively
• A positive attitude and a compassionate approach to nursing
