    The Independent Living Nurse Navigator serves as a trusted clinical resource for residents living independently within the Life Plan Community. The Nurse Navigator promotes health, wellness, and successful aging by providing nursing assessment, education, care coordination, resource navigation, advocacy, and transition support. Working collaboratively with Social Work Navigators, chaplains, wellness staff, and healthcare providers, the Nurse Navigator helps residents understand their healthcare needs, navigate complex medical systems, and access appropriate services while supporting their ability to safely remain independent whenever possible.

    This position focuses on education, coaching, coordination, and early intervention rather than providing ongoing skilled nursing care or replacing

    What you'll do:

    • Serve as a resource for residents seeking information on healthcare services, insurance, and community resources
    • Provide referrals to specialized healthcare professionals when necessary
    • Coordinate and facilitate support groups for residents dealing with specific health issues like diabetes, heart disease, and mental health challenges
    • Provide one-on-one educational consultations to residents and their families about health-related matters
    • Maintain an up-to-date directory of local health care providers, support services, and other relevant resources
    • Perform any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor 

    What you'll need:

    • Bachelors or Masters prepared nurse 
    • Minimum 2 years of care coordination/case management experience 
    • Strong organizational skills
    • Experience with geriatric population

    The compensation for this role is $83,500–$85,000 based on background and experience.
