We are seeking to hire a Nurse Navigator for our Boise Oncology Clinic!

As an Nurse Navigator in the Saint Alphonsus Medical Group, you will deliver the course of care and treatment as determined under the leadership of the Oncology Inter-Disciplinary Team (Oncologists, Specialists, Surgeons, Pathologists, Radiologists and Genetics) or other service line utilizing nurse navigation. You will navigate the patient's care collaboratively across multiple disciplines, departments, and providers within the system to achieve seamless patient care. Includes the assessment and provision of all psychosocial resources necessary to support the patient and their family.

Schedule: Monday, Tuesday, Wednesday 7am-4pm)

SKILLS, KNOWLEDGE, EDUCATION AND EXPERIENCE:

  • A Colleague must have an RN license, as defined by their primary work state (Idaho or Oregon), issued before the hire date. *ID Only: Multistate licenses require establishing residency with the Idaho Board of Nursing within 60 days of hire

  • All colleagues must provide licensure or proof of application in process for the secondary state within 90 days of hire.

  • A bachelor's degree in nursing is strongly preferred.

  • 2 years of RN experience required.

  • At hire, Basic Life Support certification from the American Heart Association or American Red Cross is required. (ACLS and/or PALS may be accepted in lieu of BLS)

ESSENTIAL FUNCTIONS:

  • Knows, understands, incorporates, and demonstrates the Organization's Mission, Vision, and Values in behaviors, practices, and decisions.

  • Provides education, emotional support and direction as determined by the physician and APP leadership of the intra-disciplinary team to specific patients. Communicates and coordinates the plan of care with the patient.

  • Works as the patient's advocate necessitating a seamless continuum of care. Involves family/guardian in plan of care and conducts patient and family education as appropriate. Explores the patient's understanding and knowledge of current health status.

  • Ensures patient has full access to all of the services and support resources as needed. (e.g., financial advisement, nutrition education, counseling support).

  • Demonstrates quality management in alignment with program requirements and/or other process improvements of the service line and participates in quality improvement process initiatives as it relates to this program.

  • Develops standards, policies, procedures, and protocols to support the Navigator Program.

  • Performs essential collection and dissemination of information for all financial resources, pre-authorization, clinical trial adherence and grant opportunities as needed by the patient. Works closely with patient accounts to assure accuracy of charges.

  • Participates in data collection to integrate fiscal efficiency with high quality and continuity of care and provides monthly/quarterly statistical data and reports or input to help identify areas for process improvement.

  • Maintains continuing education necessary to keep competent in current trends of care in the clinical and diagnostic setting. Meets all annual mandatory hospital competencies, as well as the clinical and equipment competency checklists for the position and patient age group.

  • Provides communication to multidisciplinary care team about the patient's plan of care, as determined by each service line: Educates the PCP on the role of the Navigator

Keeps the PCP in the loop of the treatment plan

Provides seamless "handoff" of the patient back into their primary care post
