Short Description:

The Cardiac Nurse Navigator serves as a primary clinical resource for patients throughout their continuum of care.

This role provides patient education, triages clinical concerns, coordinates care across multidisciplinary teams and promotes positive patient outcomes through effective communication and collaboration.

The successful candidate will also provide cross-coverage for other Nurse Navigator roles within the EP team to ensure seamless patient care.

 

  • Triage incoming patient phone calls and assess clinical needs appropriately.
  • Provide timely patient education regarding cardiac conditions, procedures, medications, and follow-up care.
  • Identify device alerts in the remote monitoring system for cardiac implantable electronic devices to assist physicians and advance practice providers in managing patients with heart rhythm disorders
  • Coordinate care across physicians, advanced practice providers, nursing staff, and ancillary departments.
  • Conduct in-office patient interactions, including education, care coordination, and follow-up visits.
  • Cross-cover additional Electrophysiology Nurse Navigator roles as needed to support departmental operations and ensure continuity of patient care.

 

Experience Required:

Cardiology nursing experience required. 

Education Requirements:

BSN required, MSN preferred. 

License/Certification Requirements:

Current NJ-RN License.

Special Requirements:

 

  • Electrophysiology and/or cardiac device management experience preferred.
  • Experience caring for patients with pacemakers, implantable cardioverter-defibrillators (ICDs), cardiac resynchronization therapy (CRT) devices, and implantable loop recorders (ILRs) preferred.

 

Salary Min ($): USD $37.00 Salary Max ($): USD $61.00
