Nurse Navigator - RN

POSITION PURPOSE AND SUMMARY The Neuroscience Nurse Navigator is responsible for creating, developing, and managing the post-discharge navigation program for the Neuroscience service line. With a broad knowledge base in neuroscience disease prevention, diagnosis, and treatment, this role delivers comprehensive clinical care coordination across the care continuum with a primary focus on the critical transition period between hospital discharge and outpatient follow-up. Working in collaboration with the multidisciplinary healthcare team and in partnership with a non-clinical Health Navigator, the Neuroscience Nurse Navigator provides navigation program coordination and oversight, clinical surveillance, patient, family, and caregiver education, and evidence-based resources to facilitate informed decision-making, medication adherence, reduction in readmission, and secondary prevention. This role coordinates disease-specific programs aligned with organizational and governing body objectives, guides the multidisciplinary team in standardized evidence-based care delivery, and maintains oversight of programmatic performance, regulatory requirements, and quality/data reporting. EDUCATION/EXPERIENCE: Licensed Registered Nurse licensed in Indiana. BSN required; MSN preferred Strong Neuroscience/Stroke knowledge. Stroke Nurse certification preferred SCRN or CNRN Minimum two years experience caring for stroke/neuroscience patients BLS ACLS certification. Excellent communication skills. Excellent patient assessment skills including assessment of psychosocial issues. Knowledge of self care resources and strategies Strong data skills Critical thinking skills Strong leadership skills Basic knowledge of insurance reimbursement systems, financial hardships, and payer coverage Working knowledge of national, regional and community resources Able to work independently and collaboratively with members of the healthcare team. Experience with EMR and Microsoft Word Products. Demonstrates a professional, positive, flexible demeanor. Resourceful and adaptable to the changing healthcare climate. Valid Drivers License and automobile insurance for travel to various sites.
