POSITION TITLE: Nurse Post Discharge Coordinator
PAY RANGE:
LVN/LPT $35.00-44.19 Per Hour
RN $54.00-68.18 Per Hour
REPORTS TO: Director of Quality Assurance and Performance Improvement.
DESCRIPTION OF POSITION:
In collaboration with multidisciplinary team, provides quality services across discharge planning, crisis calls, referring to community-based resources, data collection, and documenting the patient’s medical status post-discharge. The Post Discharge Crisis Coordinator will follow the expectations of the Zero Suicide protocol per Corporate’s guidelines, and will also support in a hybrid role the hospital’s nursing department. Serves as a member of interdisciplinary team supporting the organization's treatment program and philosophy, and assures the deliverance of quality treatment to patients and their support systems.
