POSITION SUMMARY

• Perform clinical interventional and episodic direct care visits, and conduct necessary re-evaluations of care visits.
• Review medications, orders, and diagnostic tests, and address clinical abnormalities appropriately for all patients under their care, in accordance with the permissions and/or requests from collaborating physicians.
• Continuously evaluate and treat chronic, co-morbid illnesses as deemed medically suitable.

QUALIFICATIONS

• Valid Nurse Practioner or Phyisician Assistant License in the state of Pennsylvania, and current board certification in your field.
• BSN Degree or Higher Required
• Demonstrated expertise in Nursing and the practical application of critical thinking skills specific to Nurse Practitioner roles. Ability to apply these skills effectively to the care of elderly, chronically ill, and recovering residents.
  
• Strong leadership, interpersonal, and problem-solving abilities are preferred.
• Excellent written and oral communication skills are essential.
• Punctuality, organizational skills, and the ability to work effectively within a team are crucial traits.
