NURSE PRACTITIONER (NP) - IN-HOME HEALTH RISK ASSESSMENTS

• Position: PRN / 1099 ContractorExclusions:PMHNP, Women's Health, Pediatric NP's and PA's.• Schedule: Flexible (50+ hours/month)• Compensation: $100-$115 per visit (Tiered based on availability)• Territory: Cuyahoga and Lorrain Counties

We are seeking Nurse Practitioners (Certain NPs only; no PAs) with no less than 40 hours per month who can start immediately completing in-home Health Risk Assessments (HRAs) for Medicare Advantage members. Must be able to complete credentialing paperwork, training, and onboarding within the first 10-12 days and start field-based work immediately after thereafter.

This is a pure assessment role: No prescribing, no on-call, and zero follow-up care.

THE PAY BREAKDOWN

• 60 hrs/mo: $100 / visit
• 70 hrs/mo: $105 / visit
• 80 hrs/mo: $110 / visit
• 90 hrs/mo: $115 / visit
• Additional Pay: $250 flat-rate paid training, $25 full HRA no-shows, $65 diagnostic-only visits, $10 diagnostic no-shows.Fast-Track Bonus: Extra incentive bonus if you complete onboarding paperwork within 24 hours!

• Active NP license (No PAs; most specialties accepted except Pediatrics, PMHNP, and Women's Health).
