Overview of the Role:
Our client is seeking a compassionate and skilled Nurse Practitioner to join a dynamic team within the PACE Health Clinic and Day Center. In this impactful role, you will provide both direct and indirect care to participants, working under the guidance of the Medical Director/PCP. You will have the opportunity to manage complex clinical situations, facilitating high-quality care for a vulnerable population. Your contributions will directly support the health and well-being of elderly patients in a nurturing environment.
Responsibilities:
Conduct health histories, physical exams, and patient teaching with precision.
Order, interpret, and follow up on lab tests, ensuring comprehensive documentation in the EMR.
Initiate emergency measures and identify abnormalities for immediate referral.
Evaluate and adjust medication and treatment orders per established protocols.
Coordinate and manage participant care daily in collaboration with the attending physician.
Liaise with physicians, nurses, and healthcare professionals, fostering educational communication.
Participate in the PACE provider call schedule for cohesive team support.
Actively engage as a member of the Interdisciplinary Team and contribute to care planning.
Document and communicate participant changes, ensuring informed team collaboration.
Evaluate and treat participants in various settings, including homes and healthcare facilities.
Mentor medical students and residents during their PACE rotation.
Educate participants on Advanced Directives and discuss DNR orders with physicians as needed.
Requirements:
Master's degree in clinical nursing from an accredited Nurse Practitioner program.
3-5 years of nursing experience, with a preference for Nurse Practitioner experience.
RN licensure in respective state and Family certification from a Nurse Practitioner program.
Authorized to practice within the designated scope.
Meet state and CMS-approved competencies for the role.
One year of experience with frail or elderly populations preferred; training provided if needed.
Reliable transportation, valid driver’s license, and state-required liability auto insurance.
Up-to-date immunizations and medical clearance for communicable diseases.
Successful completion of a comprehensive criminal background check.

Ability to work autonomously in clinical and management decisions.
Detail-oriented with excellent organizational skills.
Strong verbal and written communication proficiency.
Flexible and adaptable to varying work schedules.

Effective in physical assessment and disease management.
Competent in basic computer applications and team collaboration.
