    Nurse Practitioner – Dialysis Services

    Location: San Antonio, TX and surrounding areas JobType:Full-time

    Position Summary

    South Texas Renal Care Group is seeking a dedicated and experienced Nurse Practitioner (NP) to provide comprehensive care to patients receiving dialysis across multiple outpatient dialysis unitsinSanAntonioandsurroundingcommunities.TheNPwillcollaboratecloselywith nephrologists, nursing staff, and interdisciplinary teams to optimize patient outcomes, manage chronic kidney disease (CKD) and end-stage renal disease (ESRD), and ensure high-quality, patient-centered care. This role is focused exclusively on dialysis units with no outpatient clinic or hospital rounding responsibilities.

    Key Responsibilities

    • Provide routine and acute clinical care for patients undergoing hemodialysis and/or peritoneal dialysis.
    • Conduct initial assessments, follow-up visits, and monthly dialysis rounds.
    • Evaluate, diagnose, and manage AKI, CKD, ESRD, and dialysis-related complications.
    • Collaborate with supervising nephrologists regarding patient care plans and clinical decision-making.
    • Review and act on lab results, dialysis adequacy metrics, and medication regimens.
    • Manage comorbid conditions such as hypertension, diabetes, anemia, and fluid/electrolyte imbalances.
    • Prescribe medications and adjust treatments in accordance with established protocols and stateregulations.
    • Participate in care coordination with social workers, dietitians, and dialysis staff.
    • Ensure compliance with CMS, ESRD Network, and facility quality standards.
    • Educate patients and families regarding disease management, dialysis options and transplant considerations, treatment adherence, diet/fluid management, and lifestyle modificationstoslowCKDprogression.
    • Document patient encounters accurately in the electronic medical record (EMR).

    Schedule & Coverage

    • Travel between multiple outpatient dialysis centers in San Antonio and surrounding areas (driving required).
    • Flexible weekday schedule; early mornings may be required based on dialysis shift times.
    • No weekend call responsibility.
    • No outpatient clinic or hospital rounding responsibilities; role is dedicated to dialysis units.

    Qualifications

    • Current Texas Nurse Practitioner license (or eligibility to obtain).
    • Board certification as an NP (ANCC or AANP).
    • DEA license and prescriptive authority in Texas (or eligibility).
    • Minimum 1–2 years of clinical experience preferred but not required; dialysis, nephrology, or internal medicine experience strongly preferred but not required.

    Preferred Experience

    • Prior experience in dialysis, nephrology, or hospitalist medicine.
    • Familiarity with ESRD quality measures and CMS regulations.
    • ExperiencewithEMRsystems.
