A fast-growing long-term care organization is seeking a compassionate and dedicated Nurse Practitioner to join its team at a nursing home in Edison,NJ. This is an excellent opportunity for an experienced provider who is passionate about delivering high-quality primary care to a geriatric patient population in a supportive and collaborative environment.
Responsibilities:
- Provide comprehensive primary care services to nursing home residents
- Perform new admission evaluations and ongoing follow-up visits
- Monitor, assess, and manage acute and chronic medical conditions
- Collaborate with interdisciplinary care teams to ensure optimal patient outcomes
- Document patient encounters thoroughly and accurately in the medical record
- Support continuity of care and contribute to overall quality of care within the facility
Requirements:
- Active Nurse Practitioner license in the state of New York
- Active DEA license
- Minimum 6 months of experience working as a Nurse Practitioner
- Prior nursing home or long-term care experience strongly preferred
Compensation & Benefits:
- Highly competitive salary: $150,000–$155,000 annually
