Job Overview

We are seeking an experienced Nurse Practitioner for a flexible, part-time opportunity providing primary care and post-hospital follow-up services throughout the Winchester, Virginia area. Care is delivered primarily in patient homes and senior living environments.

The Winchester need can be filled independently at approximately 1-2 days per week. A Nurse Practitioner interested in providing coverage in both the Winchester and Culpeper service areas may work approximately 3 days per week.

Qualified Family Medicine or Internal Medicine physicians (MD/DO) who are interested in this type of part-time mobile care opportunity may also be considered.

Patient visits are geographically coordinated each day. Providers will see patients who are located near one another rather than being expected to drive throughout the entire service territory in a single day.

Role Overview

The provider will care for patients requiring ongoing primary care as well as follow-up after hospitalization. Responsibilities include assessing acute and chronic conditions, conducting post-discharge evaluations, developing treatment plans, prescribing and coordinating appropriate care, and working closely with patients, caregivers, and other healthcare professionals.

Key Responsibilities

  • Deliver primary care and post-hospital follow-up services in patient homes and senior living communities
  • Perform comprehensive assessments and ongoing clinical evaluations
  • Conduct post-discharge visits to support safe transitions from hospital to home or senior living
  • Evaluate, diagnose, and manage acute and chronic medical conditions
  • Develop individualized care and treatment plans
  • Prescribe medications and order appropriate diagnostic testing within scope of practice
  • Coordinate referrals and collaborate with physicians and other members of the care team
  • Educate patients and caregivers on medications, disease management, preventive care, and treatment plans
  • Monitor patient progress and modify treatment plans as clinically appropriate
  • Complete required EMR documentation within 24 hours of each patient encounter
  • Consult with clinical leadership when additional guidance or collaboration is needed

Schedule

  • Winchester coverage anticipated at approximately 1-2 days per week
  • Providers interested in covering both the Winchester and Culpeper areas may have approximately 3 days per week available
  • Flexible part-time scheduling
  • Typical hours are 8:00 AM-5:00 PM
  • No on-call responsibilities
  • Orientation and onboarding are provided
  • Credentialing generally requires approximately 30 days

  • Coverage is centered around Winchester, Virginia, with patients located throughout:

  • Frederick County
  • Shenandoah County
  • Clarke County
Additional Service Area (Optional for 3 days a week)

Daily routes are organized geographically so that patients scheduled on the same day are located close to one another. Providers will not be expected to travel across the entire service area during a typical workday.

Required Qualifications

For Nurse Practitioner Candidates:

  • Active Virginia Nurse Practitioner license
  • Current DEA registration
  • Active CAQH profile and CAQH number
  • At least one year of Nurse Practitioner experience in primary care, family medicine, senior care, assisted living, long-term care, or transitional care
  • Experience using electronic medical record systems
  • Strong clinical assessment abilities and confidence making independent care decisions

For Physician Candidates:

  • Family Medicine or Internal Medicine MD/DO
  • Appropriate active Virginia medical licensure and prescribing credentials
  • Relevant experience providing primary care or managing medically complex patients
  • Ability to work effectively in a mobile, independent care environment

Preferred Qualifications

  • Post-acute or transitional care experience
  • Background in primary care, urgent care, emergency medicine, hospital medicine, or acute care
  • Experience caring for geriatric or medically complex patients
  • Previous experience delivering care in patient homes, assisted living, or other senior living settings
