Nursing Home Nurse Practitioner - NYC Boroughs

Location: Manhattan base (10018), serving nursing homes across NYC Boroughs

Position: Nurse Practitioner - Family Practice

Provide specialized primary care to nursing home residents across NYC boroughs (Manhattan, Bronx, Brooklyn, Queens, Staten Island) from a Manhattan hub.

Key Responsibilities:

• Conduct assessments, manage chronic conditions, and med reconciliation in SNFs
• Order labs/imaging, treat infections/wounds, and coordinate hospice/palliative care
• Collaborate with facility staff, MDs, and families for care plans
• Complete regulatory documentation and quality metrics

Qualifications:

• FNP or AGNP certification with active NY NP license
• Geriatrics/long-term care experience preferred; new grads welcome
• Reliable transportation; BLS; Spanish bilingual a plus
