About the Role:
• We are seeking an experienced Nurse Practitioner (NP) to provide comprehensive primary care services to a diverse patient population in the Bronx.
• The ideal candidate will have strong experience in primary care, along with exposure to or experience with infection control/infection prevention and substance use disorders.
• This position is ideal for an NP who is comfortable working independently, managing a broad range of patient needs, and collaborating with an interdisciplinary care team.
Key Responsibilities:
• Provide comprehensive primary care to adult patients, including evaluation, diagnosis, treatment, and follow-up.
• Perform physical examinations, obtain medical histories, and assess acute and chronic health conditions.
• Develop and implement evidence-based treatment plans.
• Order, review, and interpret diagnostic testing and laboratory results.
• Prescribe medications and provide ongoing medication management.
• Provide patient education, preventive care, and health counseling.
• Identify and address patients with substance use disorders and coordinate appropriate treatment and community resources.
• Apply knowledge of infection control and infection prevention practices within the clinical setting.
• Coordinate care with physicians, nurses, behavioral health professionals, and other members of the interdisciplinary team.
• Maintain accurate and timely documentation in the electronic health record (EHR).
• Participate in quality improvement initiatives and maintain compliance with FQHC, regulatory, and clinical standards.
• Provide culturally sensitive care to patients from diverse backgrounds and help address barriers to accessing healthcare.
Qualifications:
• Current New York State Nurse Practitioner license and national board certification.
• Master’s degree in Nursing or related field required.
• Strong clinical experience in primary care.
• Experience with or knowledge of infection control/infection prevention practices required
• Experience working with patients affected by substance use disorders/addiction strongly preferred.
• FNP, ANP, or comparable NP certification.
• Bilingual Spanish Preferred
• Experience serving underserved, multicultural, or community health populations preferred.
• Prior FQHC or community health center experience is highly preferred.
• Ability to work independently while collaborating effectively with a multidisciplinary team.
• Strong clinical judgment, communication, documentation, and organizational skills.
• Proficiency with electronic health records.
